T.I.D.E. 2010 Required Forms for ALL T.I.D.E. Registrants to Complete Prior to Arrival at T.I.D.E.

Northeastern University staff requests that you bring photo identification with you at check-in. Photo ID may be a driver’s license, state ID, school ID card or a passport. If you do not have a photo ID, please bring identifying paperwork (for example an official copy of your birth certificate) and a recent (school) picture with your name on the back.
ALL check-in is done at White Hall, 21 Forsyth Street, Boston, MA. Plan to check-in on Friday between 4 and 6 PM. Daytime attendees also need to arrive by 8AM on Saturday and Sunday to check-in for the day.
Nightly check-out times for non-overnight attendees is with IFA staff and occurs at 8:30 PM (prior to nightly special events) or at 10 PM (following the conclusion of the special evening events). The conference concludes on Sunday, May 30 at 5 PM. Daytime attendees should plan to sign out with IFA staff and depart at 5PM. Overnight guests will have from 5 to 7 PM on Sunday to pack their belongings, check-out of their rooms, return their keys and sign-out.
Live drop-off and pick-up is advised. Parking is available at the Renaissance but it is paid parking and it is a good walk across campus to White Hall. Public transportation is advised for Boston area attendees.
Every attendee, whether teen or adult, must download, complete and return the following:
- Either the TIDE Teen Health and Consent Form if you are a teen younger than 18 by May 28, 2010 or the TIDE Adult Health and Consent Form if you will be 18 years of age or older by May 28, 2010.

We strongly urge all attendees to scan and email back these two forms to Interfaith Action as soon as possible. No attendees may check-in and participate at the T.I.D.E. 2010 Conference without both of these completed signed forms on file with Interfaith Action. While you may bring them with you, it would be best to email them back to Interfaith Action ahead of time.
